The following forms are required by the State of Maryland. They must be submitted to Mother of God School before your child may attend classes. 

Download and complete one set of forms for each child that will be attending school this fall.

State Mandated Forms 

1) Emergency Contact Form *NEW FORM FOR 2023 (both pages must be filled out by parent – please answer all questions – use N.A. or None if not applicable)

2) Health Inventory Form * Part I must be filled out by parent; Part II must be filled out by your child’s Pediatrician

3) Dental Health Card * must be filled out by Dentist

4) Immunization Form* must be filled out by your child’s Pediatrician

5) Lead Testing Form * Box A and B must be filled out by parent/guardian; Box C and D must be filled out by your child’s Pediatrician.
