The following forms are required before a student may attend school. We request all forms be turned into Mother of God before August 7, 2023. 

Download and complete one set of forms for each child that will be attending school this fall.

2) Health Inventory Form * Part I must be filled out by parent; Part II must be filled out by your child’s Pediatrician

3) Immunization Form* must be filled out by your child’s Pediatrician

4) Lead Testing Form * Box A and B must be filled out by parent/guardian; Box C and D must be filled out by your child’s Pediatrician.
